South Australia mandates medicine bar coding
Bar code scanning has become mandatory for all medicines in South Australia, a practice that will significantly
cut down on dispensing errors and related adverse drug events, the SA Pharmacy Board says.
"Analysis of medication errors indicates that prevention strategies targeting systems rather than individuals are most effective in reducing errors," said a recent e-bulletin released by the pharmacy department of SA's Repatriation General Hospital.
The statement said 11% of medication errors in hospitals occur during the dispensing phase and that "statistics
indicate that the majority of dispensing errors involve selection of the incorrect strength of a medicine or selection of the incorrect product".
The Australian Commission on Safety and Quality in Health Care advocates the use of electronic medication management
(EMM) systems for reducing medication error and a growing number of Australian hospitals are looking towards
implementing EMM systems to optimise medication safety, the November bulletin said, adding that bar coding officially became the rule for all SA pharmacies and pharmacy departments in July of this yea
